There are several types of treatments to get pregnant that are indicated depending on the case. Ovulation induction is a treatment used for cases where a woman has been diagnosed with anovulation, women with hormonal imbalances, PCOS, LUF, or unexplained couple infertility. This is the most commonly used fertility treatment by gynecologists because it can be done with simple oral medications like Clomid, Indux, Serophene, and others, as well as injectable inducers based on gonadotropins.
Timed intercourse is an extension of ovulation induction. It enhances this procedure with tests to detect the exact day of ovulation. The ideal day for intercourse will be determined by the size of the follicle, which ruptures at about 21mm, measured through serial ultrasound exams. The chance for a couple to get pregnant with this procedure, without further complications, is 35% according to age—the younger the couple, the easier it will be to achieve the desired pregnancy.
Artificial insemination (AI) will probably be the first recommendation from a fertility specialist for the couple. It consists of inducing ovulation with medications, just as in timed intercourse. This process is complemented by the collection of the man's semen, followed by a washing to balance the pH and select the best sperm, and then carrying out intrauterine insertion into the woman on her most fertile day. Artificial insemination, besides being less invasive than in vitro fertilization, is a more affordable procedure. It costs about R$1,500 to R$3,000 including procedure and medication.
In vitro fertilization (IVF) is a step ahead of artificial insemination and just before the most modern technique, ICSI. It has become one of the fastest-growing treatments to get pregnant, both in demand and recommendation. It is usually chosen by couples who have previously tried all other possible methods to conceive and have not been successful. IVF involves inducing ovulation, aspirating the eggs, and collecting the male material, semen.
The selection of the best eggs and sperm is done based on maximum potential, joining them via test tube. A needle introduces the sperm into the nucleus of the egg, and then specialist doctors follow the growth and development of the zygote's cells. After five days, the best embryos are transferred to the woman’s uterus. In most cases, the couple, together with the doctor, decides how many will be transferred. However, two or three embryos are usually transferred, if possible.
ICSI and IVF procedures are very similar. What differentiates them is the stage after collecting eggs and sperm. ICSI involves inserting the sperm into the nucleus of the egg with a very fine needle. This needle is about eight times thinner than a hair strand. ICSI and IVF are recommended for couples with low gamete production or poor quality gametes. IVF costs around R$12,000 to R$20,000 for procedures and medication.
There are places that offer this procedure for free to couples with an indication for fertility treatment. In major centers like São Paulo, it can be done at the USP Clinics Hospital, Ribeirão Preto Clinics Hospital, Unifesp Hospital, or the Santa Casa de São Paulo. Some locations limit to two or three attempts, others have no limits. However, the waiting time is considerable, around two years to undergo the procedure.
There are also projects that can be done at a reduced cost for both medication and the process. They can go from R$4,000 to R$8,000 depending on the region, but still much less than the normal price if done through private clinics.
