Study: Health and Getting Pregnant – Diet, Weight, and Physical Activity

Difficulties getting pregnant are affecting more and more Brazilians. According to the Brazilian Society of Assisted Reproduction, 8 million people are unable to achieve this dream. The causes of infertility are diverse, but among them, there are some factors couples themselves can influence. These include diet and physical activity. But do these factors actually have an impact on fertility?

To answer these questions and assess the extent of the problem, Trocando Fraldas conducted a survey with more than 5,000 women who have had or currently have difficulties getting pregnant throughout Brazil, between August 6 and 9, using a questionnaire. The participants responded voluntarily, indicating age, place of residence, and family situation, which made it possible to compare by region and state.

Questions

The following questions were asked:

  • How long did it take you to get pregnant or how long have you been trying?
  • On a scale from 1 to 5, how healthy do you consider your diet when trying to conceive?
  • On a scale from 1 to 5, how healthy do you consider your partner’s diet when trying?
  • On a scale from 1 to 5, how would you rate your weight while trying?
  • On a scale from 1 to 5, how would you rate your partner’s weight while trying?
  • During attempts to conceive, on how many days per week do/did you eat fruits and vegetables?
  • During attempts to conceive, on how many days per week does/did your partner eat fruits and vegetables?
  • During attempts to conceive, on how many days per week do/did you engage in physical activity?
  • During attempts to conceive, on how many days per week does/did your partner engage in physical activity?

Results

To compare groups, regions, and states, responses were converted into indexes and applied on an individual scale. For diet in general and overweight, the denominator used was 2, considering 3 (healthy and normal weight) as reference values. For fruit and vegetable intake and physical activity, the national average served as the reference denominator.


General Diet and Getting Pregnant

  • The average assessment from mothers, pregnant women, and those trying to conceive indicates their diet during attempts was or is 15% below what they consider healthy
  • Diet quality improves with age, with women aged 40 to 44 rating their diet as 27% healthier than women aged 18 to 24
  • Mothers and pregnant women who took less than 3 months to conceive have a diet 29% healthier than women who have not succeeded within that timeframe. The same applies to their partners, though the difference is only 12%.
  • Concern for diet among couples increases during the first 12 months of trying and decreases after, but never reaches the level of those who conceive in 3 months or less.
  • Partners’ diet is considered 5% better than women’s, but is still not healthy and diet quality also increases with age
  • Diet is best in the South region and worst in the Central-West among couples trying to conceive

Weight, Overweight, and Getting Pregnant

  • 1 in 3 women considered their weight above or well above normal during attempts, a trend that increases with age from 28% in the 18 to 24 age group to 43% above 40 years
  • The length of time trying is related to overweight, with only 31% of those trying for up to 3 months above normal weight and 40% among those trying for 18 months or more
  • Only 3 in 10 mothers who had no trouble getting pregnant were above normal weight, but this is true for 3 out of 8 with fertility problems
  • Among partners, obesity at 28% is less common due to the lower rate among young men (23%), but increases quickly with age, reaching 43% over age 40
  • The relationship between men’s weight and difficulty getting pregnant is not as consistent as it is for women
  • Women trying to conceive outside the Southeast have a higher obesity rate and those in the North have the lowest, but are still on average above normal weight.

Fruit and Vegetable Intake During Attempts

  • Women trying to conceive eat fruits and vegetables on average only 2.87 times per week, but only 6% eat them every day and 1 in 8 doesn’t include them in their diet at all
  • Consumption increases with age, being 23% higher among couples over 40 trying to conceive compared to those aged 18 to 24
  • Fruit and vegetable intake increases in the first year of attempts, drops in the second year, and rises again only in couples trying for more than 24 months
  • Regardless of how long they've been trying, healthy food consumption is always higher among women who managed to conceive, with the biggest difference seen in those trying for less than 3 months at 18%
  • Partners consume less natural vitamins at only 2.54 times per week, with the same age-related trend
  • Partners’ intake of fruits and vegetables in childless couples and those with trouble conceiving is lower than in couples with children

Physical Activity During Attempts

  • Partners of women trying to conceive do sports on average twice per week while their partners only manage 1.4 times, with activity decreasing with age
  • Half of women trying and 39% of partners do not engage in any weekly physical activity
  • Physical activity among couples who succeeded in conceiving after difficulties is 10% higher compared to those still trying to get pregnant
  • Couples trying to conceive in the South are the most active, while Northeastern men and Southeastern women are the most sedentary
